Dear All Out member,
We are writing to inform you about a recent issue that may have involved information about you. Our records indicate that earlier this year you completed an All Out survey. We recently discovered that an All Out service provider inadvertently made information from this survey publicly accessible on a cloud-based data storage website. The information may have included your e-mail address and survey responses. We don't believe that this information could be misused for identify theft or fraud, but we nonetheless wanted to bring the issue to your attention. We deeply regret that this issue occurred and take very seriously the privacy and security of information about our members.
When we learned of the issue, we immediately took steps to ensure that the information was completely removed from the data storage website. We also have terminated our relationship with the service provider. In addition, we are examining measures that we can take to prevent issues of this nature in the future.
Because the information involved included e-mail addresses, you should be cautious of potential spam or suspicious messages. Specifically, you should be cautious of any e-mails coming from an unknown source, and you should never give out any personal or financial information.
Once again, we apologize for this issue. We care deeply about the trust that members place in our organization. Please see below for answers to some common questions regarding this issue. If you have further questions, please do not hesitate to contact us at email@example.com.

What data was included in the survey?
The data included e-mail address and answers provided to survey questions, including sexual orientation and past experience participating in LGBT advocacy.
Do I need to be concerned about identity theft or fraud?
The information involved did not include your Social Security number or other personal information commonly used for identify theft or fraud, such as financial information. As a result, while you should always remain vigilant, we do not believe that it is likely that the survey information could be used to steal your identity or commit fraud.
Is there anything I need to do to protect myself or my personal information?
As always, you should be wary of unsolicited e-mails or any communication from a source you do not recognize. Do not reply to any e-mails or click on any links in e-mails that come from an unknown sender, and never provide personal information via e-mail to anyone.
All Out is bringing people together in every corner of the planet and of every identity – lesbian, gay, straight, bisexual, transgender and all that's between and beyond – to build a world in which everyone can live freely and be embraced for who they are.
